Internship Proposal Form – 395 or 495

Make sure you are choosing the correct proposal form before moving forward. This form is for 395 or 495 course registration.
If you are participating in an undergraduate research internship, return to the Forms section and use the UGR Proposal.

Use this form as you DRAFT and REVISE the details related to your internship with your Faculty Sponsor. Once everything has been finalized, go to Handshake > Career Center > Experiences > Request Experience, enter the information from the form as prompted, and attach the Proposal Form as a Word document.

Once submitted to Handshake, you can track your approvals there!
