Internship Offer Form

Employers:
Thank you for extending an internship offer to a Berea College student. Please complete this DocuSign form based on your conversations with the student, which will then prompt a signature process by email. Once you and the student have signed, it will be routed to the Internship Office for review, and you will both be prompted to download a copy of the completed document for your records (view sample Offer Form here). If questions arise, you will be contacted.

Students:
This form is to be filled out by the employer making you an official internship offer (for a sample email to send to your employer requesting this, click here). Once they fill it out, you will be prompted to sign it. Once you’ve signed the Offer Form, it will be routed to the Internship Office for review, and you will be prompted to download a copy of the completed document for your records. If any changes need to be made, the Internship Office will reach out to you directly. Once the Offer Form is completed and accepted by the Internship Office, you will be given access to submit your Proposal in Handshake.
